There are two forms of fitness certificates one can receive:

General fitness certificate – A paper that ascertains that the person involved is fit. The fact must state there is no major physical and mental problem. A customized fitness certificate – Most organizations have a model fitness certificate available that any person can print themselves for reference. Once that certificate is printed, it is mostly presented to a registered medical practitioner who would see the individual and sign the certificate. Usually, the registered medical practitioner carries out a few basic tests and then decides how fit they are. Else, a person can himself get examined at a diagnostic center. He can then furnish the diagnostic reports to the doctors in question. In addition to those diagnostic reports, the individual has to provide a customized fitness certificate in order for the analysis procedure itself to be complete. Based on the elaborate evaluation, a signed and sealed fitness certificate from the registered medical practitioner will subsequently be issued subject to the fact that the individual shall be found in good health condition.
